Go to your Documents tab located in the dashboard on the left.
Click the Paper Airplane Icon to the right of the assessment you’d like to send.
Select your Patient's name from the recipients drop down list.
Add a subject, for example, "Please Complete Intake Before Next Session", and content to the message if you wish.
Click Send. This will send a message to your Patient where they can save the form and complete it in their Documents folder. They will see all forms you have sent them in their Assigned Forms tab. Once complete, they will automatically be directed to send the form back to you which you will receive via a message attachment.
You will click Save to Patient File next to the attachment in the message. This completed document will then be visible within this Patient's Patient file under Patient List.
